How COVID-19 Lockdowns Left Deep Marks on Teen Brains, Bodies, and Emotions
The COVID-19 pandemic was a profound disruption to adolescent development. New research now shows that the lockdowns did not only impact teens' mental health, but may have fundamentally altered their developmental biology, leaving lasting changes in their stress responses, immune function, and brain activity.
Research Approach
A team led by Justin P. Yuan and Ian Gotlib compared two groups of adolescents:
One group assessed before the pandemic (from 2016–2019),
Another assessed after major lockdowns ended (October 2020–March 2022).
The groups were matched by age, sex, socioeconomic status, early life stress, and pubertal development to ensure that observed differences were primarily due to the pandemic experience.
The study focused on three biological systems sensitive to chronic stress:
The HPA axis, especially daily cortisol levels (the body's primary stress hormone).
Immune activation, measured via C-reactive protein (CRP), which signals inflammation.
Brain response to emotional and reward stimuli, especially in the prefrontal cortex (PFC), which plays a big role in decision-making, emotion regulation, and thinking ahead.
Key Findings
Adolescents who experienced the pandemic lockdowns showed notable differences compared to their pre-pandemic counterparts:
Reduced total cortisol output throughout the day, suggesting a dampened stress hormone profile.
Elevated systemic inflammation, as indicated by higher CRP levels, typically seen in individuals exposed to prolonged stress.
Altered brain activity, with decreased activation in the prefrontal cortex during tasks involving emotional regulation (labeling emotional faces) and reward processing.
Put together, these changes resemble those previously observed in adolescents who’ve experienced chronic adversity or significant early life stress.
Significance and Unknowns
The impact of these findings on long-term health is profound, especially since adolescence a period when brain, body, and behavior are extremely plastic. Such changes could shift developmental paths in ways that affect long-term health. Increased inflammation, for instance, is a known risk factor for mood disorders, cardiovascular disease, metabolic issues, and more.
It’s hard to say whether the changes are permanent, or partially reversible as social life and routines normalize. We don’t yet know whether teens bounce back biologically when stressors are lifted, or whether some alterations stick for years.
Bigger Picture: What This Means for Teen Health
The findings reinforce that mental health is only part of the picture. The pandemic’s toll on adolescents extends much deeper — into the physiological systems that lay the groundwork for adult well-being.
Interventions matter: Societies, schools, and health systems should prioritize support not just for psychological resilience, but for physical and biological recovery: stress‐management, access to healthy social environments, physical activity, sleep, nutrition.
Monitoring is critical: Since early biomarkers like cortisol dysregulation, inflammation, and altered brain activation can predict later illness, we should establish long-term monitoring and care for this cohort.
Inequality is a concern: As the study sample was relatively privileged, teens in more precarious circumstances likely faced even greater risk.
Final Thoughts
If this study teaches us one thing, it’s this: the lockdown experience for teens was not just about isolation and missed classes. It was a full-bodied stress event — one that may have reset some of their biological systems. We’re only beginning to understand what that looks like, and what it will mean years down the line.
As life returns to normal, the focus should be on helping young people rebuild not just socially and emotionally, but also physiologically, as adolescence profoundly influences their overall life trajectory.
